Supports for Community Living (SCL waiver) is a community-based program sponsored by the Kentucky Department of Medicaid Services administered by the Division of Developmental and Intellectual Disabilities (DDID). LucasaCare BG, LLC and LucasaCare East offer residential services through this waiver program. Services are based on the individual’s goals, choices, and priorities identified through a person-centered service plan.
The Residential Support Services include:
Level 1 care is provided in a staffed residence with no more than three people living together who require supervision up to 24 hours a day. Residential Supports assist with independent living skills such as personal hygiene, dressing and clothing, financial management, cooking, nutrition, personal growth, and safety.
Level 2 care is provided in a home or adult foster care setting, alongside an existing family who has chosen to provide this level of support. The Family Home Provider (FHP) provides an opportunity for individuals with intellectual and developmental disabilities to thrive within a family home environment. Participants become active members of the household as well as the community.
Respite is a residential service that is provided to individuals unable to independently administer self-care on a short term basis due to the absence or need for relief of the individual’s caregiver or adult foster care provider.
